You simply need to send your full name, year of graduation, and mailing address to Kara Moran at kmoran@indofmd.org to verify your info and sign the MVA form. We will then mail you the signed form and you submit that form with the $25 fee to the MVA. For further questions about the plates (e.g. numbers available), you must contact the MVA.
